There are several ways to apply for Hawaii Medicaid:

  • Online: Apply through mybenefits.hawaii.gov - available 24/7
  • By Phone: Call the Med-QUEST Service Center at (808) 524-3370 (Oahu) or toll-free 1-800-316-8005
  • In Person: Visit your local Med-QUEST eligibility office
  • By Mail: Download the application form and mail to your local eligibility office

Required Documents

When applying, you will need to provide:

  • Proof of identity (driver's license, state ID, passport)
  • Proof of U.S. citizenship or eligible immigration status
  • Proof of Hawaii residency
  • Social Security numbers for all household members
