There are several ways to apply for Hawaii Medicaid:
- Online: Apply through mybenefits.hawaii.gov - available 24/7
- By Phone: Call the Med-QUEST Service Center at (808) 524-3370 (Oahu) or toll-free 1-800-316-8005
- In Person: Visit your local Med-QUEST eligibility office
- By Mail: Download the application form and mail to your local eligibility office
Required Documents
When applying, you will need to provide:
- Proof of identity (driver's license, state ID, passport)
- Proof of U.S. citizenship or eligible immigration status
- Proof of Hawaii residency
- Social Security numbers for all household members
- Proof of income (pay stubs, tax returns, benefit statements)
