Join Hawaii's Medicaid provider network and help serve the healthcare needs of over 400,000 Medicaid members across the state.
Enrollment Steps
- Step 1: Obtain a National Provider Identifier (NPI) if you don't already have one
- Step 2: Complete the Hawaii Medicaid provider enrollment application
- Step 3: Complete required background checks (if applicable)
- Step 4: Contract with one or more QUEST Integration health plans
- Step 5: Complete credentialing with your chosen health plans
- Step 6: Begin serving Medicaid members

- Advanced practice providers (NP, PA)
- Hospitals and health systems
- Behavioral health providers
- Dental providers

- Home health agencies

- Non-emergency medical transportation providers
- Laboratories and imaging centers
Enrollment Requirements
- Valid Hawaii license (or applicable state license)
- National Provider Identifier (NPI)
- DEA registration (if applicable)
- Professional liability insurance
- Background check clearance
- Completed enrollment application and supporting documents
