Submitting Forms

Completed forms can be submitted in several ways:

• Online: Through mybenefits.hawaii.gov (for member forms)
• By Fax: Check the form for the appropriate fax number
• By Mail: Send to your local Med-QUEST eligibility office
• In Person: Drop off at any Med-QUEST eligibility office
